Certification Reimbursement (Kingston Area Minor Hockey Association)

Team Officials - Please complete the form below to request a reimbursement from KAMHA for expenses related to becoming certified for a KAMHA team. When requesting a refund please note the following: 1. All requests are subject to approval and must be submitted through this website. The request must include all of the information requested below (any incomplete applications will not be processed). 2. KAMHA will only reimburse expenses related to the current season (the team official must be on a KAMHA approved team roster). 3. All receipts must be attached to be processed. Please allow six weeks to process the request.
